必要書類

1) 国籍離脱届 2通（総領事館の窓口で入手できます）

2) 戸籍謄本 1通 　

3) 日本の旅券（パスポート）

4) 身分証明書（運転免許証など、住所の記載のあるもの）又は公証人の公証を受けた居住証明書

5) 上記抄訳文 2通 

6) 外国籍を有する旨の証明書（国籍証明書、パスポート、出生証明書など）

7) 上記抄訳文 2通

8) 本人が15歳未満であるときは法定代理人の資格を証する書面及び身分証明書

9) 上記抄訳文 2通
 

注意事項

• 届書きはすべて日本語で記入して下さい。
• 届書はすべて戸籍に記載されている氏名で記入してください。
• 届出は日本国籍を離脱しようとする方の住所地を管轄する総領事館に、必ず届出人本人（または15歳未満の場合には、法定代理人の資格を証する公的資料を持参のうえ、法定代理人）が提出してください。
• 戸籍謄本には出生時から重国籍の記載のあるものが必要です。もし、その記載がない場合は別途補足資料が必要な場合がありますので、あらかじめご了承ください。
• 法務省において国籍離脱の手続きが完了すると（手続きには2、3カ月かかります）、国籍離脱通知書が交付されます。

PROCEDURE IN RENUNCIATION OF JAPANESE NATIONALITY BY DUAL NATIONALS

• To renounce Japanese nationality, a person of Japanese-American shall submit the following documents to the Japanese Consulate:
  • 1. a) Notification form to be written in Japanese except where English writing is specifically required.
    1. b) Notarized affidavit signed by the renunciant certifying to the fact of American citizenship.
    1. c) The Koseki Tohon issued by the municipal office of the renunciant’s permanent domicile in Japan.
    1. d) A copy of passport or certified copy of birth certificate.
    1. e) Proof of address (Driver’s License or mail showing your name and address).
    1. f) All English documents should be translated into Japanese.
• If a renunciant is 15 years of age or over, he/she will sign the Renunciation Notification form in the Japanese writing under the item designated “todokede nin”, placing right thumb imprint after the signature.
• If a renunciant is a minor, under 15 years of age, both parents or legal guardian should prepare the affidavit of citizenship for the child and sign the notification papers.
• One copy of the Koseki Tohon will suffice if there are more than one member of the family renouncing Japanese nationality at the same time.
• If a woman is married and such marriage is recorded in her husband’s Koseki Tohon but no mention therein of her birth in the United States, it will be necessary for her to attach an additional Koseki Tohon of her original family.
• If the birth dates or the names recorded in the Koseki Tohon and in the birth certificate differ in any way, additional notarized affidavit must be attached declaring that the different dates or names refer to the same person. Such affidavit must be prepared by either the parents or elder close relatives who are well acquainted with the renunciant.
• If the Renunciation Notification is accepted and final action is taken by the Japanese Ministry of Justice, a procedure which takes approximately two to three months, the renunciation will be so notified.
• The renunciatnt must appear in person to submit the forms.
